New Patient Referrals

Thank you for your interest in referring your child or patient to our practice for their neurological care needs. We are constantly working to expand services and availability in our practice that will enable us to better serve our patients.

We have board-certified child neurologists who specialize in stroke, neuromuscular/movement disorders, headaches/migraines, epilepsy/seizures, neurocutaneous disorders, sleep medicine, autism/cognitive disorders, and mitochondrial disorders. Additionally, we have a board-certified pediatrician who specializes in autism and cognitive neurology.

Our Comprehensive Pediatric Epilepsy Program offers coordinated scheduling of outpatient clinic visits, diagnostic testing, hospital admission to the Epilepsy Monitoring Unit (EMU), evaluation for surgery, as well as pre- and post-surgical follow-up care.

Same day urgent appointments are available five days a week for new patients who need to be seen urgently (by peer-to-peer physician referral only).


For Referring Physicians/Clinics

Before we can schedule your patient with a pediatric neurologist, we will need the following records faxed to us for professional medical review.

  • Physician order with the reason for referral
  • Completed referral form (see Pediatric Consultation Center Referral Form) to include patient demographic and insurance info with referring physician’s contact information
  • Office/clinic notes (last 2 visits)
  • All labs within the past 12 months
  • Brian Imaging Reports (Films/CDs may be requested Prior to appointment at Neurologist discretion)
  • All EEG, EMG, EP, PSG reports
  • ED/Hospital Discharge Summary reports

For Patients & Families

Once your child’s referral has been reviewed and accepted by our team, you will be contacted by a representative from our centralized Referrals and Consultation Center to schedule a new patient appointment.

Once the appointment is scheduled and confirmed, you will receive a confirmation letter via US mail or electronic mail (whichever you prefer). Along with the confirmation letter, you will receive a Welcome Handout with information about our policies and procedures, as well as, a New Patient Information Form. Please complete this form and bring it with you to the first clinic appointment.
